pa: Use the FMA rtx code. 

        * config/pa/pa.md (fmadf4): Rename from unnamed; use FMA.
        (fmasf4, fnmadf4, fnmasf4): Likewise.
        (unnamed plus+mult insns and splitters): Delete.
